1230 A Case of Intratonsillar Abscess Presenting as a Non-Resolving Acute Tonsillitis With Dysphonia, With Learning Points

Abstract Peritonsillar abscess (quinsy) and tonsillitis are two commonly diagnosed treated pathologies in ENT departments. However, intratonsillar is rarely reported medical literature textbooks, therefore understanding of when to clinically suspect such pathology likely less known amongst junior doctors. We present a case healthy adult for large abscess, at district general hospital the United Kingdom. A 27-year-old man with one previously quinsy, presented 6 days worsening sore throat, fevers, dysphonia. Clinical examination showed mild trismus, an asymmetrically right tonsil, appearance peritonsillar swelling, normal neck exam. Clinically this was initially as needle aspiration space, however 3 attempts revealed no pus. He admitted cellulitis by antibiotics, but over four little improvement seen rising inflammatory markers. Particularly “hot-potato voice” dysphonia did not improve all. Given smoothly enlarged tonsil absence suspicion raised. Needle into 7 millilitres thick pus, causing immediate voice symptoms. suggest that could be suspected patients appearing quinsy dry attempts, or unilateral unresolving tonsillitis. Direct can sometimes avoid radiation from CT scanning who systemically well deep infections.
